  1. Print out the order form.
  2. Please fill out the name to appear on the tag(s)
  3. The team name will be the name we used on the drop down menu for viewing the team pictures. You do not need to include the jersey #.
  4. Check off the item(s) you would like to purchase and place the total in the total box. All applicable taxes are included in the price and add ($2.00 for shipping/handling) for any size order.
  5. FIll out the address and mail payment along with the order form to Custom Signs and Grafx Works.

BAG TAGS Our tags are approximately 4.25 " in length x 2.5" in width,laminated with a special clear UV coating to protect against fading and come with a plastic loop for fixture onto a bag. Front of Tag will have the player name on the front. Back of Tag will have the team roster.
